As part of a combination therapy with procarbazine and vincristine (PCV regimen), Lomustine is the standard of care in most lower WHO grade gliomas with IDH mutation (oligodendrogliomas, astrocytomas). In addition, Lomustine is the main standard of care for recurrent glioblastomas.
Lomustine
Lomustine is a nitrosourea and spontaneously decomposes into a chloroethyl carbonium ion via the chloroethyldiazonium ion after oral administration. The cytostatic effect is due to the alkylation of the O6 of guanine by the chloroethylcarbonium ion. In addition, bifunctional alkylation (by two reactive sites) leads to cross-linking of DNA and induction of apoptosis. Lomustine can penetrate the blood-brain barrier and is therefore suitable for the treatment of brain tumours.
